A longevity clinic can spend real money on content, an SEO retainer, a blog, a content writer, and still see almost no organic bookings, while the handful of patients who do call say they found the clinic through ChatGPT or Perplexity, not Google. Both problems trace back to the same root cause. Generic wellness content doesn't rank and it doesn't get cited, because neither Google nor an AI answer engine can tell your page apart from the thousand other clinics using the same four adjectives about "optimizing healthspan."

Every longevity clinic website says some version of the same sentence: this protocol supports healthy aging, boosts energy, or optimizes cellular health. That language communicates nothing specific, and search engines and AI systems alike have gotten good at recognizing it as filler. A patient who has spent weeks researching NAD+ or peptide therapy can tell the difference instantly between a page that explains a real biological mechanism, in language only someone who understands the underlying biology would use, and a page recycling marketing copy. So can the ranking and citation systems trained on millions of examples of both.
What actually gets cited by ChatGPT and Perplexity
A 2026 measurement study analyzed 602 controlled prompts and more than 21,000 valid citations across ChatGPT, Google's AI Overview, and Perplexity. Perplexity and Google cite broadly, 16 and 12 sources per prompt on average. Slapping an FAQ block on a thin page is not a citation strategy.
A separate 2026 study tracking over 100,000 AI answers across 100+ brands found that only 2.9 percent of AI citations point back to a brand's own domain, the other 97 percent is third-party coverage, and that niche, small brands appear in roughly 11 percent of relevant AI answers on their first tracking run, compared to 73 percent for globally recognized names. One page per protocol, NAD+, peptide therapy, hormone optimization, whatever the clinic actually offers, written with real mechanism-level specificity: what it does biologically, exact candidacy criteria, what gets monitored, and what outcome measures the clinic actually tracks. Benefit language belongs in the intro paragraph for the human reader. The substance belongs in the body, for the search and citation systems reading for evidence.
Attribute every page to a named, credentialed practitioner
Cross-link every clinical page to a real bio with real credentials. This is both an E-E-A-T signal for Google and an authority signal for AI systems trying to decide whether a clinic is a trustworthy source to cite for a health-adjacent answer.
Get the local foundation right before chasing AI citations
None of this replaces Google Business Profile optimization and local SEO fundamentals. Skipping straight to "AI visibility" content while the clinic still isn't showing up in the local map pack is solving the wrong problem first.
Write for evidence density, not for an FAQ template
Comparisons, real numbers, named studies, and clear procedural steps are what the citation research found actually gets absorbed into AI answers. A generic FAQ section formatted like a template does not move that needle on its own.
Refresh protocol content on a real cadence
Treatment and protocol details change. A page from eighteen months ago describing outdated dosing or a discontinued test panel doesn't just look stale, it stops being useful evidence for either a search engine's freshness signals or an AI system checking whether a source still reflects current practice.

My take
The "SEO vs. AEO vs. GEO" framing sold to wellness and longevity clinics is mostly repackaging. The underlying discipline is the same for a classical search engine and an AI answer engine: specific, well-structured, evidence-backed content, credibly attributed to someone who actually knows the subject. A clinic does not need a separate budget line for "AI visibility." It needs to stop publishing generic wellness copy and start publishing content that reads like it was written by someone who understands both the biology and the business behind it. A lot of the agencies selling "AI citation" packages to this exact audience are charging a premium for access to the same content quality bar Google already rewards.
